Felix, a fit, 20-year-old construction worker from Dandenong, was part of a 10 man crew working on a large infrastructure project, rebuilding a flood-damaged bridge in East Gippsland. The crew stayed in shared on-site cabins (3 people per cabin) by the river for three weeks, they mostly had their evening meals outdoors and Felix was able to do some fishing in the evenings after knock-off. 5 days after returning home from the Gippsland job, Felix became unwell; his symptoms included fever, chills, painful and swollen knee and ankle joints, and a rash of small raised red spots on his back and chest. The doctor diagnosed Ross River Fever and blood tests confirmed that the infectious agent was Ross River Virus (RRV).
RRV is an alphavirus, a small positive-sense single-stranded RNA virus that is transmitted via mosquito bites. The virus and the disease are named for the Ross River in Queensland and RRV cases are most common in Northern Australia and Papua New Guinea, however cases are increasingly being recorded in Southern states of Australia. It is thought that marsupials such as kangaroos and wallabies act as a reservoir for the virus. Once in the human host, the virus infects muscle cells where it replicates rapidly before spreading via the blood to synovial cells, triggering painful inflammation of the joints. The known incubation period for RRV is 3-7 days, however it can take as long as 21 days before symptoms appear. Most people will recover from infection within 3-4 weeks, however 10-20% develop chronic disease with debilitating muscle and joint pain persisting for up to 6 months. Studies in Papua New Guinea, where the virus is endemic, suggest that having high levels of IgG antibodies specific for RRV envelope protein E2 can confer long lasting protective immunity to RRV, but there is no vaccine or antiviral treatment that is effective against RRV. Treatment consists of Analgesics and anti-inflammatory medications to ease the symptoms.
