Angry Adolescent
A large majority of my psychiatric mental health experience has been with children and adolescents. I have worked with a large number of adolescents that experience all four behaviors as shown in the case studies. I have selected the case study of the “angry adolescent” for this discussion board.
This adolescent projects on the counselor with statements of “you are a horrible counselor,” “I hate being here,” “I hate talking to you,” “this is worthless,” “you are stupid,” and “you know nothing about me” (Laureate Education, 2013). This young female displays angry emotions towards the counselor about the session. A possible diagnosis for this patient would be oppositional defiant disorder (ODD) according to the DSM-5. The DSM-5 criteria describes oppositional defiant disorder as angry/irritable, often argumentative to authority figures, refuses to comply with requests, often blames others, and vindictive at least twice in the past 6 months (American Psychiatric Association, 2013).
Therapeutic approaches are available with an adolescent and ODD. Methods include parenting education, social-emotional skills training, and medication use. Parenting education takes a look at the relationship between the parents/guardians and the child. The best approach for ODD is helping the parent learn new parenting strategies to create structure and consistency. The social-emotional skills training will aid the child in identifying their feelings, how to have better relationships, and decision-making skills. Some effective medications are stimulants for ADHD, antidepressants for depression and anxiety, and other medications. Some medications include Ritalin, Dexedrine, Lexapro, Prozac, Tenex, Risperdal, and Abilify (Boston Children’s Hospital, 2020).
Early identification and early treatment of ODD will have a better opportunity for improvement/changes. Behaviors associated with ODD can be difficult and take some time to alter. Studies have shown that 67% of children with ODD were symptom free after approximately three years. Many are likely to develop other conditions such as ADHD, anxiety, or mood disorders. Also, about 10% of children develop a personality disorder late in their life. Therefore, early treatment is essential in a child with ODD for the best expected outcomes (AACAP, 2019).

The hyperactive child is an adolescent, African American, Female client. Her speech is rapid, pressured, and tangential. Her thought process is disorganized with flights of ideas. She appears impulsive with poor boundaries as evidenced by grabbing items that do not belong to her. She presents with psychomotor agitation evidenced by constant fidgeting in her seat, grabbing objects, and manipulating them continuously in her hands. The client expresses an aversion to doing homework and reports that her parents are always “on my back about video games” (Laureate Education, 2013). While this video file does not provide the ability to ask important follow-up questions for determining diagnostic criteria, the data that can be extrapolated is highly suggestive of ADHD. The client’s observed behaviors are consistent with the following DSM criteria for ADHD: Distracted by external stimuli, frequent fidgeting, constantly on the go, talking excessively, and frequent intrusive or interruptive behavior. Follow-up questions regarding her dislike for homework might meet DSM criteria for inattention to detail in homework, difficulty organizing tasks, and reluctance to engage in activities requiring prolonged mental effort. Together, these inattentive, impulsive, and hyperactive symptoms exceed the six required for a diagnosis of ADHD (American Psychiatric Association, 2013).
The client would be initiated on Methylphenidate SR 18 mg each morning. Stimulant medications are first-line pharmacologic agents for the treatment of ADHD in adolescents, as they display the highest efficacy for both impulsive and inattentive symptoms and improve long term outcomes in those with ADHD (Stahl, 2013). The most evidenced therapy for ADHD is behavioral therapy, in which the therapist works with parents to modify the child’s environment and help improve behaviors. This is done via behavioral parent training in which strategies such as operant conditioning are taught in order to modify dysfunctional behaviors and reinforce positive behaviors (Nathan & Gorman, 2015). The intended outcome for both pharmacologic and nonpharmacologic interventions is to reduce symptoms in order to improve goal-directed behavior. This will ultimately result in improved scholastic achievement, better interpersonal relationships, and will thereby improve long-term functioning overall.

According to Spath (2018), FOCUS PDCA is a frequently used quality improvement model used to improve processes. This change model has two phases. The first phase, FOCUS, consists of finding a process needing improvement, organizing a team, clarifying the current process and areas needing change, understanding why improvement is needed, and selecting actions to adjust the process (Spath, 2018). The second phase, PDCA, includes planning through investigation, identification, and brainstorming ways to improve the measure, do or incorporate the process improvement on one unit, check the process to make sure changes are improving the measure, and acting to maintain improvements (Spath, 2018). This model provides the structure through analysis, planning, outcomes, and feedback.
FOCUS PDCA is a change model that can be used to decrease falls. Falls during toileting is one safety concern that needs improvement. The quality team, along with other care team members, were organized to clarify current practices and policies and noted care team members are not staying within arms-reach of patients that trigger high fall risk. It is imperative to keep an open line of communication between the team and frontline staff (Godlock, Christiansen, & Feider, 2016). Understanding the policy deviations and brainstorm to select appropriate interventions to decrease falls during toileting. Interventions that need to be changed requires education to all care team members about the falls policy and how important it is to stay within arms-reach of patients that trigger high fall risk. Education also needs to be provided to CNA’s on certain medications that would make patients higher risk like blood pressure-lowering medications, and diuretics. These interventions will be rolled out to one unit to determine the efficacy of changes. These interventions need to be reviewed for improvement and modified if necessary. Once improvement has been met, it is crucial to maintain these improvements. Maintaining improvements can be accomplished through monthly quality improvement meetings.
Discussion 2:
I selected the quality improvement model of FOCUS-PDCA. This model is split up into two separate phases the FOCUS phase and the PDCA phase. F is for finding a problem that need improvement. Falls can be a detrimental occurrence than can affect patients and staff that is involved. Nursing times stated that as many as 20% of hospitalized patients experience a fall at some point during their stay (Grant, 2013). Even though falls are not always preventable they can be decreased with proper staff and patient education. If one fall can be prevented by a changed policy that can be one less patient that avoids an extended hospitalization or additional injury. O if for organizing a team. The team I have selected for the improvement project is my nurse manager and mentor as well as some floor nurses from both shifts and our patient care techs. In the next phase, P is for plan to change the process. In a study by Lipsett patients falls were decreased by color coded mobility cards being placed in each patient room depending on how they required assistance (Lipsett, 2019). This would be much easier to see and associate with then just a generic fall risk or no fall risk. D is to do the change on a small scale. We will pilot the fall risk mobility cards on our unit. We have a mix of post op, post-partum, and medical surgical patients. C for checking the data to determine its effectiveness. We will compare the falls for the next months after the implementation with the fall rate prior to implementation. Lastly is A to act to maintain the gains. According to Hayes, Chapman, Young and Rittman (2015), chronic disease affects the lives of family members causing physical, emotional, social and economic problems. Caregivers are at risk for caregiver role strain due to increased obligations and responsibility related to the role of caregiving. Many studies show that caregivers often neglect themselves in the process of caregiving. According to Yigitalp, Surucu, Gumus, and Evinc (2017) primary caregivers reported high levels of stress and moderate levels of social support when giving care to patients living with a chronic disease. Furthermore, Collins et al., (1994) defines caregiver burden as experiences of negative outcomes such as psychological distress carried by the care giver who takes on the burden of care. Changes and frustrations in the lives of caregivers and there family can cause the care taker experience somatic and psychosomatic problems. Instruments such as questionnaires, scales and indexes have been developed to measure and assess caregiver role strain and perceived social support. For example, in 1980, Reever and Bach-Peterson developed the Zarit Care Giving Burden Scale (ZCGBS) to evaluate stress levels of caregivers of individuals requiring care (Yigitalp et al., 2017). At the start of care, the nurse must observe communication pattern with the patient in their family to assess knowledge about disease, and note attitudes and behavior during the care giving process. This will help the nurse focus on caregiver responsibilities and develop appropriate interventions for the complete care plan.

Diabetes is a serious public health problem throughout the world, affecting people of all ages. When uncontrolled, diabetes can place drastic limits on daily life, and is sometimes fatal.
Blood glucose is your main source of energy and comes from the food you eat. According to the NIH, Diabetes is a disease that occurs when the blood glucose is too high (2016). Insulin is a hormone produced by the pancreas to regulate the blood sugar, sometimes the body does not produce enough insulin, or the body does not utilize the insulin well. There are three different types of diabetes, type 1, type 2, and gestational. With type 1 diabetes, the body does not produce insulin and the individual has to use insulin to manage their blood sugar; type 2 diabetes the body does not produce enough insulin; gestational diabetes is developed by some pregnant women with chances of going away after having the baby, but increases the risk of type 2 diabetes in the individual (NIH, 2016)
Healthy People 2020 goals to control diabetes is to reduce the disease burden of diabetes mellitus (DM) and improve the quality of life for all persons who have or are at risk for diabetes. Also reduce the annual number of new cases in the population and reduce the rate of death among people with diabetes. Healthy people 2020, the CDC, in conjunction with many other organizations are working together to accomplish all the objectives by providing funds to train medical personnel which ensures proper treatment, assessments, and education about management of diabetes are available to the community (Healthy People 2020, 2020).

The ACA or the AHCA: Imperfect Solutions to Increasing Access to Health Care?
This week’s discussion focuses on the federal government’s role in protecting the public’s health using the ACA and current efforts to “repeal and replace” the ACA. Please note that the government’s role in protecting the public’s health did not begin with the ACA. In the late 1800s, Lilian Wald brought health care to the community. And in 1965, Congress passed bills authorizing Medicare and Medicaid. Since 1965, Congress has expanded government-sponsored health care to pregnant women and children.

The Affordable Care Act is also know as Obamacare. This act was created with the intent to help get insurance to those that were uninsured. According to Mason (2016), “the ACA provisions aim to increase access to care; change the culture of health care from one of cure to one of health promotion and illness prevention; mitigate barriers to practice for primary care providers of all disciplines; capitalize on the skill and expertise of nursing in areas of leadership, practice, research, and innovation; and, through these mechanisms, improve population health outcomes” (p. 193). Many felt that the AHCA would cause many of the previously insured Americans to lose insurance coverage. With many people losing coverage, this would create a problem in the poor community with the ability to have access to health care. The AHCA failed in its first attempt due to lack of proper planning. Although I feel that there are parts of the ACA that need change, we have to fully analyze the effects that this would have on the consumers. I personally feel that the government needs to step in on the reimbursement rates that insurance companies have to pay. I currently work and live in one of the poorest counties in South Carolina. We have one hospital in the entire county. I have had to wear many hats working at Williamsburg Regional Hospital. One of these hats include investigating the reimbursement of insurance versus the cost of the procedure. We have had to stop performing procedures due to the simple fact that the reimbursement rate does not cover the cost to the hospital.

Healthcare reform has made several efforts by the federal government earliest date known was 1984, but was not considered successful until 2010. ACA created the Center for Medicare and Medicaid Innovation that was intended to facilitate value-based approaches to healthcare primarily by creating and piloting alternative payment models (Hirsch, Rosenkrantz, Nicola, Harvey, Duszak, Silva, Barr, Klucznick, Brook & Manchikanti, 2017). ACA has two goals in mind such as increasing the number of insured individuals, and reducing the cost of healthcare. These goals were met by expanding Medicaid and creating the “exchange” marketplace (Hirsch et al., 2017). As this bill was intended to help the uninsured, it actually increased the deductibles and out of pocket expenses for those with the exchange insurance and employer based plans (Hirsch et al., 2017). AHCA has mentioned that individual plans, the premiums would be based on age with a 5% increase for elderly as ACA has a 3% increase. In my opinion, the reason ACHA has failed is because of the increase in the population that lost insurance and had no additional insurance or means to pay. Although, it really seems like both bills want to point the finger at each other and say that it is because of “pre-existing conditions” or “stipulations imposed” in the bill that caused the failure. Regardless of the “why”, we have to figure out some way to benefit each American that is cost effective and provides the much needed healthcare coverage.
