Use this form to develop your Individual Success Plan (ISP) for each course. An individual success plan maps out what you, the DNP Learner, needs to accomplish in order to be successful in your current course and overall program of study. You will also share this with your mentor at the beginning and end of each course so that they know what you need to accomplish.
Application based learning assignments are listed in the course syllabus with a Practice Portfolio Statementrequirement. This statement also serves as a reminder for you to complete a weekly corresponding Case Log in Typhon. Any independent study assignments in this course requires your collaboration with the course faculty and practice mentor early on to establish a plan for successful completion of mutually identified and agreed upon deliverables (proof of completion) and must relate to your current course objectives. Independent study assignments must be submitted to your instructor via the Individual Forum if no drop box is available.
In order for you to successfully complete and graduate from the DNP Program you must meet the following programmatic requirements: (1) completion of required practice immersion hours, (2) completion of work associated with all DNP and GCU program competencies, (3) successful defense and completion of your Direct Practice Improvement Project.
In this ISP, you will identify all of the objectives, tasks, and/or assignments relating to the programmatic requirements you need to complete by the end of this course (minimum 50 practice immersion hours are built into the curriculum for each course). Specify the dates by which you will complete each tasks and/or assignments. Your plan should include a self-assessment of how you met all applicable GCU DNP Domains & Competencies (see Appendix A). This information will be required as you enter your weekly Case Log.
ALL course assignments listing a ‘Practice Hours Portfolio’ statement must be included in the ISP and are worth and recorded here as approximately 10 hours each. Actual clock hours are recorded into Typhon. Note that required practice hours vary from learner to learner and is determined by your MSN transcripts. Please contact your SSA for the number of practice immersion hours required to complete your DNP program. This will assist you in creating a customized ISP and practice hours plan for your entire DNP program of study. Some learners will require more than the 50 practice immersion hours built into the coursework. It is your responsibility to obtain this information and create a plan for your individual success.
Students who require more than 50 practice hours per course will need to create additional independent study assignments that target the course objectives. You have the option to repeat a course assignment using a different scenario/context or create one based upon the current course objectives with your mentor and faculty input. Because practice settings and career goals vary, this allows the learner to customize and make the most of their learning experiences while in the DNP program at GCU.
General Requirements
Use the following information to ensure successful completion of the assignment as it pertains to deliverables due in this course:
Review the DNP Program Milestones document in the DC Network listed under the tab DPI Project Resources> DPI Milestones Guide and identify which milestones apply to this course. Note: Not all courses have milestones. Please see the DNP Program Timeline for a quick overview. Recall that all pertinent and up-to-date DNP program materials are in the DC Network.
Determine what practice experiences you plan to seek in order to address each course objective. Include estimated number of associated practice immersion hours. Learners will apply objectives and concepts from each of their courses to reflect upon, critically examine, and improve current practice and are required to integrate scholarly readings to develop case reports that demonstrate increasingly complex and proficient practice.
Use the Individual Success Plan to develop a personal plan for completing your practice immersion hours and self-assess how you will meet the GCU DNP Domains & Competencies(Appendix A) related to that course. Show all of the major milestones (if applicable) and deliverables. ISPs do not earn practice hours, nor does telephone conference time, or time spent with mentor.

Expert/Entrepreneur Interview: As part of this process, you will need to conduct an Expert/Entrepreneur Interview in advance of doing your elevator pitch. The purpose behind this interview is to allow you to find out more about your business idea. You may choose an entrepreneur already working in this field or an expert in your technology or sector. There are many people to consider for your idea. For example, if your idea is to develop a new type of crutches, then you might want to interview someone from a medical devices company, a physical therapist, a medical device retailer, an entrepreneur who has brought a product to market, someone who has patented an idea…. Additionally, this assignment allows you to network with an “expert” or an “entrepreneur” which may help in your future job search, internship, etc. Ideally, the interview should take place in person, but if there are circumstances (e.g. interviewee is not in the area) then Skype will be allowed. Things to Consider when Conducting and Setting Up Interview
You should refer to your idea in very general terms, rather than use the interview time to describe your idea because your ideas are still very, very early. You might begin the conversation by saying something such as “I am in an entrepreneurship course and I am considering developing a product for use by individuals who have a leg injury—some type of crutch maybe. The idea is not yet well-developed and I am hoping to be able to visit with an expert in this area….”
Call prospective individuals early. Most will be willing to meet, but have very busy schedules.
If you choose to “tape” the entrepreneur make sure that you have his/her permission to do so at the beginning of the recording.
Post-Work
Within a week of conducting the interview, you should send a handwritten thank-you with a promise that you will follow up with your paper. This gives you an opportunity to make several different contact points: 1) setting up the interview, 2) conducting the interview, and 3) written thank-you note. These contact points are ways in which to reinforce the relationship and create a strong impression. The search terms included “nurses,”“healthcare workers,”“hand hygiene,”“handwashing,”“alcohol-based hand hygiene,”“healthcare-associated infections,”“alcohol sanitizer,” and so forth. After completing the search, many sources were identified. However, it was the duty of the research to select only those that met the inclusion criteria. For example, Asadollahi et al. (2015) discussed the levels of knowledge of nurses concerning the concept of hand hygiene and the need for training while Awoke et al. (2018) addressed the issue of hand hygiene compliance, which was found to be poor leading to the need for adopting measures aimed at improving the adherence levels. Conversely, Luangasanatip et al. (2015) examined the efficacy of different interventions to promote hand hygiene, while Oyapero and Oyapero (2018) assessed hand hygiene perceptions among nurses, which is similar to Sharif et al.’s (2016), which explored hand hygiene perceptions and attitudes in nurses. Therattil et al. (2015) had credible evidence on different hand hygiene methods as White et al. (2015) sought to assess the determinants of hand hygiene decisions in nurses, while Zhao et al. (2018) provided evidence on how to improve the attractiveness of hand hygiene interventions. Therefore, this credible evidence revolves around promoting hand hygiene to mitigate HAIs.
Application of the Change Theory
Kurt Lewin’s theory of planned change is appropriate for this change proposal. Its details are indicated in Appendices A and B. Batras et al. Consequently, collective planning through a team-based approach will help in designing protocols and measures to improve hand hygiene compliance. This plan will then be actualized during the implementation phase before evaluations of the effectiveness to achieve high adherence levels, reduce the occurrence of HAIs in the ICU, the duration of ICU stay, and improve the associated health outcomes, among others.

Introduction: Social Capital and Community Organizations
So far in the class, we’ve defined organizations and community. We’ve thought about how sociological paradigms relate to community organizations and that theory is the basis for research. Next, we discussed research and the distinction between quantitative and qualitative studies. We took this conversation a bit further and then saw how community-based organizations can be driven by research and the important role of applied sociologists in helping assess and evaluate programs and community.
Now, we’re going to move forward and talk about social capital. In our modern society, we are all connected to one another. With the Industrial Age and the decrease of agriculture, people began to live closer to each other. Our networks grew and we saw the importance of these relationships that existed outside of the family. Social capital is a relatively new term that explores the value of these relationships.
Consider, for example, that you are looking for a new job now that you have graduated with your master’s degree. You are interested in working for a community organization. Where would you start your job search? Often, we start with the internet, but then we move on and think of people we know. You may call your cousin, who works for the local county. She may refer you to a friend, who works for a youth-oriented nonprofit, who then asks you to come in and talk about your ideas and expertise. Perhaps this meeting will lead to a job, or to a referral to another organization! This network is valuable; social capital is a powerful concept that continues to grow with the increased use of social media and the multiple ways that people are connected to one another.

Go to a local department store to gather data. Walk through the toddler clothing departments and take careful note of the differences in clothing available for boys and girls. FIELDWORK OPTION B: Social Class in the Supermarket
Go to a supermarket in one of the more affluent areas of town, and then visit a supermarket in one of the poorest sections of town. (Note: you must actually visit the stores for this assignment, even if you have been there before. You will miss a great deal if you simply rely upon your memory.) On your notepad, make detailed observations about the condition of the stores. Although you are free to observe whatever details you think are important, at a minimum you should address the following questions:
o How is the cleanliness of the stores?
o How is the politeness of the employees?
o Are there security guards or visible security systems? If so, what are they like, and how do you think their presence (or lack of!) affects the customers’ mental states?
o What range of items is available in the store? For example, do they sell only canned goods, or do they also have produce, dairy, toiletries, hardware, videotape rentals, etc.?
o What are the prices like? Compare the prices on several identical items.
o How fresh are the produce, fruits, vegetables, milk, and meat?
o What other things do you observe that differentiate the social class statuses of the neighborhoods in which the stores are located?
For this project, you should be creative and observant. Draw from the readings (particularly the chapter on social class, although you may find some useful information in the race/ethnicity chapter as well). Present your findings about social class in a sociological manner (that is, you should use important and relevant sociological terms to describe your findings).
Your paper should be about 750-1,000 words. Turn in your detailed fieldnotes along with your paper.
FIELDWORK OPTION C: Race on Primetime Television
In recent years, some critics have claimed that people of color are not very visible on primetime television – and when they are, they are not often presented in positive roles. Watch a sample of primetime shows for one week (you should watch at least 5 hours of primetime television on the major networks) and systematically keep track of the race of the characters, as well as the ways they are portrayed. (Note: you will actually have to watch television and take notes for this assignment – do not rely upon your memory of these T.V. shows. If you rely on your memory, you will miss a great deal.) Before starting this assignment, you will have to decide what “primetime” means (be sure to explain this in your paper). You will also need to figure out how to gauge someone’s “race” (you will need to explain this in your paper as well). In your paper, you should address the following:
o How many characters of the different races were portrayed?
o How important were the different characters to the plot/storyline?
o How were the characters portrayed – positively or negatively; stereotypical or unstereotypical? What were their roles? What did they do?
Be descriptive. Present your findings in a sociological manner (that is, you should use important and relevant sociological terms to describe your findings).

FIELDWORK INSTRUCTIONS: Follow these instructions when working on your project (i.e. these instructions apply to EVERY fieldwork option!)
This is a guide for you as you work on your fieldwork. Please read this carefully before engaging in your fieldwork.
1) The fieldwork assignments require that you look at the world around you. You should do your best to look at the world with a scientific eye. In other words, try your best to be objective, and rely on the actual evidence you see, not on your guesses or assumptions. Do NOT enter into the fieldwork with expectations about what you’re going to find or learn.
2) Do NOT wait until the last minute to begin this project. You will want to make your observations for the project at least a week before the assignment is due.
3) You must make observations for this assignment. Do NOT rely on your memory of these things – you will miss a great deal and your grade will probably suffer. In other words, just because you’ve seen children playing before, this does not mean you can write a good, detailed, analytical paper about it.
4) As you make your observations, you should have a pad of paper and pencil/pen with you. It is an excellent idea to prepare the notepad for notetaking before you begin. Write the date, time, and location at the top of the pad. Next, read the assignment carefully, making a table with headings, rows, and columns that correlate to the components/questions of the assignment. These preparations will help organize your notes and will make your observations much more easy and accurate (REMEMBER: your notes will be graded!). It will also help you answer the assignment completely. Since you will be submitting your notes to me electronically, you will need to type your notes in an organized and systematic manner (tables are frequently very helpful). I will grade your notes on their degree of organization, detail, and depth.
5) While you’re making observations, be as thorough as possible in your note taking. Be detailed! Jot down all of your observations, ideas, and hunches. You won’t be able to include everything that’s written in your notes in the final paper, but you want as much detail as possible in your notes. I can’t stress enough how important extremely thorough notes are. Students who have excellent notes almost always have excellent papers!
6) After you’ve finished making observations, review your notes. Search for themes and patterns. Do your best to make sense of what you saw. You are free to talk with a classmate (or me!) about your ideas.
7) Begin writing your paper. Your introduction paragraph should describe what you did, and it should begin to “frame” your paper in sociological thought and/or concepts. (What did you observe? Where did you go? What did you watch or read? What is the purpose of your project? Did you make any important decisions that might affect your findings? What key sociological ideas does this project relate to?) The body of your paper should describe what you saw and should address all of the questions in the assignment. (What patterns did you find? And how do those patterns relate to the course concepts? Be sure to discuss the readings, lectures, and films when applicable.) Your conclusion should wrap-up your paper. (What did your observations teach you about society and sociology?)
8) Be sure to present your observations and findings as clearly as possible. You might decide to include pictures, photographs, or a table or chart. Sometimes, visual aids help the reader “see” the patterns more easily. THIS IS NOT REQUIRED, however. If you decide to include a visual aid of some kind, it will not count toward the page requirement.
9) Citation format. In order to successfully complete this assignment, you should not refer to any additional written sources besides our course textbook. As a result, you can simply cite the textbook in the body of your paper as follows: (Newman, p.___).
10) Your paper should be typed, double-spaced, in standard font. Please do not go over 1,000 words, although I will not penalize you if you do go over the limit a little.
11) Do not forget to also submit your observation notes! It is a good idea to simply paste them at the end of your paper and submit it as one complete document.
